Update: I met with a new integrative doc. He agreed I had just about every test done, but found one that was missed. Zinc. He said he'd test it because it could very well explain a lot of my symptoms, though he didn't expect it to be low. Just got the results yesterday, and sure enough, it was low! Phew! If that would have come back normal, he was leaning towards chronic fatigue. Since I think that's a B.S. diagnosis since I clearly have things that contribute to fatigue (adrenal and thyroid deficiencies), I was not too happy about that…
I read up on zinc and found that it's critical for the conversion of T4 to T3. Um, no WONDER I did awful on T4! In addition, it's necessary for the function of over 300 enzymes in the body. I really really really hope this is my missing link and things improve with supplementation. I eat a TON of beef and chicken (I crave it all. the. time) so it looks like I'm not getting enough (or not absorbing enough) from my diet.
I hope you're making some progress, and if anything, hope you can get your zinc level checked if you haven't already!
